Funding Impact

  • The objective was to Increase access to primary care by expanding Payette Lakes Family Medicine clinic hours and implement a same-day appointment clinic model. 
  • Recruited and hires advanced practice providers (APP) (physician assistants and nurse practitioners) to staff the same day appointment clinic. 
  • Currently, we have three physician assistants and two nurse practitioners. In addition, we have two flex APP. 
  • Funds were also used to offset negative cash flow to support the Patient Centered Medical Home care model. 
  • This model is in support of our transition from fee for service to a value-based and integrated care model. 
  • The integrated care model includes staffing a team of nurse care coordinators, patient access navigators, community health outreach, licensed clinical social workers and clinical pharmacist. 
  • Recognized as Tier III Patient-Centered Medical home (highest is IV) 
  • We believe the Patient Centered Medical Home (PCMH) model is best practice and are incorporated as our guiding principles for how we operate within primary care.  
  • We continue to provide many of the required elements of PCMH in our clinics and have used the PCMH model to develop and implement programs in our clinics like embedded social workers, pharmacy, care coordination and team-based care. We found the amount of resource required to document, report, write policies, etc. to pursue accreditation was not cost effective and have discontinued efforts to pursue NCQA PCMH certification. 

Note: FY21 saw an significant increase due to COVID-19 testing and vaccination appointments 

 Patient Care Impact

  • Approximately 20 to 22 same day appointments are available daily. Calls for requests for same day appointments begin at 8 a.m. Appointments are typically filled within 30-45 minutes.  
  • Increasing access is a strategic objective of St. Luke’s, along with quality care and affordability. 
  • McCall’s family medicine clinic continues to increase access every year.  
  • From FY17 to FY22 family medicine clinic visits increased by 33 percent (9,967 visits) 
  • McCall’s clinics stepped up during the unprecedented COVID-19 pandemic to serve the community. 
  • Clinic visits increased by 25 percent (8,327 visits) from FY20 to FY21. 
  • The clinic team quickly stood up a car clinic for COVID-19 testing.  
  • Another factor effecting FY21 visits was the administration of COVID-19 vaccinations.
